Diagnosis of Pulmonary Embolus

چکیده
In patients with suspected pulmonary embolism, posterior/anterior and lateral chest X-rays are recommended as an initial imaging examination because they may reveal an alternate diagnosis and obviate the need for further imaging. If the chest X-ray is negative and the patient has a D-dimer level over an age-adjusted threshold, CT pulmonary angiography (CTPA) is the examination of choice. Ventilation/perfusion scintigraphy is recommended only for patients who are allergic to iodinated contrast media or have poor kidney function. It should also be considered in young women because the breast radiation dose is lower than CTPA. In pregnant patients with symptoms of deep vein thrombosis, compression ultrasound may yield sufficient information to initiate treatment and avoid radiation exposure. If pulmonary embolism imaging is required in pregnancy, CTPA delivers lower radiation exposure to the fetus and is the imaging modality of choice.

Cement Pulmonary Embolus Complicating Percutaneous Vertebroplasty
We present the case of a 63-year-old woman who suffered a cement pulmonary embolus that resulted from methylmethacrylate extravasation into the paravertebral venous plexus during percutaneous vertebroplasty. We discuss the radiographic diagnosis and strategies for prevention and treatment.
متن کاملPulmonary artery catheter occlusion as an indication of pulmonary embolus.
Two patients with respiratory failure had consistently reproducible findings on insertion of flow directed pulmonary artery catheters which led to the subsequent diagnosis of pulmonary embolus. Occlusion of the catheter tip and development of a rising "overwedge" tracing on deflation of the balloon may mean the catheter tip has become embedded in clot. When combined with the inability to obtain...
متن کاملAltered therapy of suspected pulmonary embolism using leg venography.
The traditional approach to the diagnosis of pulmonary embolism involves attempts to document the embolus in the lungs. An alternative diagnostic approach is to document the source of the embolus in the iliofemoral venous system. The present case shows how a search for leg thrombosis in suspected pulmonary embolism can also have an impact in choosing the optimal therapy to prevent recurrent emb...
متن کاملUnusual Presentation of A Massive Pulmonary Embolism
A pulmonary embolus clogs the artery that provides blood supply to a part of the lung. The embolus not only prevents the exchange of oxygen and carbon dioxide but also decreases blood supply to the lung tissue itself, potentially causing the tissue to die (infarct). A 52-year-old man presented with syncope and anxiety. He had sinus tachycardia and dilated right ventricle with trabeculations. A ...
متن کاملParadoxical Embolus Stuck in a Patent Foramen Ovale
In-hospital mortality rate of an embolus in transit is as high as 44.7%. In some cases, a paradoxical embolus can get stuck in a patent foramen ovale. Because of the high mortality rate, this condition should be considered as an emergency case. Echocardiography has been established as the gold standard method for the diagnosis. A negative echocardiography, however, does not rule out an embolus ...
متن کاملOlanzapine and pulmonary embolism, a rare association: a case report
Venous thromboembolism is a very common pathological process for which there are many well known (and less well-known) predisposing factors. Likewise, olanzapine is a commonly used anti-psychotic medication. We present the case of a young Somali gentleman who developed venous thromboembolic disease after an overdose of olanzapine. The diagnosis was only made 48 hours after admission, due to the ...
